Athletic Clearance
Students interested in participating in athletics at LBHS will need to complete an online athletic clearance to participate. To complete the online process, the LBUSD Physical Examination Form must be completed, signed by a doctor, and then uploaded the clearance website. If you are unable to print a copy, forms may also be picked up in the athletic office. As a part of this process, a current medical insurance card must also be uploaded. If you do not have insurance, sport-specific insurance can be purchased through the athletic office. Your athletic physical is good for one calendar year and must be done prior to trying out for another sport. When you have completed the clearance process, you will print a confirmation letter and return it to the athletic office. Important Message Regarding Athletic Clearance Physicals and Cardiac Screening Availability:
Hello Breaker Families!
We are pleased to announce the return of on-campus pre-participation exams (physicals) for athletic summer camps AND the upcoming sports season! Our team physician, Dr. Jeevan Sall, and his colleague will be on campus Thursday, May 23rd from 2:30 - 5:00 pm to conduct the exams. This event is open to all LBHS students who plan on participating in sports next school year as well as 8th grade students from TMS who will be entering their freshman year at LBHS. Cost will be $40 per exam with proceeds going towards LBHS Athletics Booster program. Additionally, there will be an opportunity to receive a full cardiac screening at an additional cost through Heartfelt Screening, a non-profit cardiac screening organization based here in Laguna Beach.
This is not a required event for student-athletes but rather an opportunity to complete the yearly pre-participation exams on campus and support the athletics programs. Student-athletes are always welcome to go to their primary care physicians or another medical clinic to complete the exams (must be completed by an MD, DO, PA, or NP). If completing the exam on campus, parents/guardians are not required to be present, but the forms must be filled out and signed by a parent/guardian ahead of time regardless of age. Pre-participation exam forms and the athletic clearance website can be found at lbhs.lbusd.org/athletics or picked up in the athletics office. Full athletic clearance is required for participation in summer camps and in athletics at LBHS.

Team Physician: Dr. Jeevan Sall is a Primary Care Sports Medicine Doctor with Providence-Mission Heritage Medical Group and has an office in Laguna Niguel. His clinical expertise includes non-operative orthopedics and sports medicine, general musculoskeletal care, sports concussion, and general medical needs of the athlete.
